All of these forms can be filled out on your PC then saved and emailed to cs@commonwealthtesting.com. The Background Check Consent form MUST be signed at Commonwealth Testing, Inc. These forms may also be printed blank and filled out by hand if you prefer.
Applicant Questionnaire - This a questionnaire asking you some particular questions about the job you are applying for and you willingness to travel to different counties.
Employment Screening Consent Form - This form must be signed in the presence of someone at Commonwealth Testing, Inc.
